Passport Application Acceptance Facility
University Mailing Services (UMS) Oklahoma State University
UMS is designated by the U.S. Department of State as an official Passport Application Acceptance Facility.
Passport applications are accepted by walk‑in or appointment, Monday–Friday, 8:30 a.m.–4:00 p.m.
Citizen Requirements
To apply for a U.S. passport, applicants must provide one of the following:
- A previous U.S. passport (expired or official, such as military)
- A naturalization certificate
- A certified birth certificate issued by the state registrar’s office
Please note: Hospital birth certificates and “notification of birth registration” documents are not acceptable.
Identification Requirements
Applicants must present one valid form of identification:
- Driver’s license (not temporary or a learner’s permit)
- City, state, or federal government ID card
- Pilot’s license
For minor applicants, the parent or legal guardian’s ID is used.
Photograph Requirements
Applicants must provide one 2x2 color passport photo taken within the last six months. The photo must be clear, front‑view, full‑face, with a plain light background, and printed on photo‑quality paper.
UMS does not take passport photos. Please obtain a 2x2 passport photo from a local provider such as Walgreens, CVS, or Walmart before arriving.
Fees
Application Fees
- Adult Passport Book: $130
- Minor Passport Book: $100
- Adult Passport Card: $30
- Minor Passport Card: $15
- Adult Passport Book & Card: $160
- Minor Passport Book & Card: $115
Acceptance Fee
- $35.00, paid separately from the application fee.
Payment Methods
Both the application fee and acceptance fee must be paid by check or money order.

Forms are also available at the UMS office: 601 North Willis St., Oklahoma State University Campus. Customer parking is available on the south side of the building.
